LifeWise Health Plan was founded in 1986 by a group of Oregon insurance professionals with a simple goal in mind. The company wanted to provide quality, affordable health plans to consumers that would be both flexible and wide-ranging. Today, LifeWise Health Plan continues the tradition of offering consumers cost-effective health plans along with superior, responsive service.


Subscribers residing in Oregon can choose from a wide range of health benefit packages and related services, timely and efficient local customer service, convenient online services, and access to a large provider network.

LifeWise Health Plan is headquartered in Portland and has its operations center in Bend. It is a privately-held, for-profit company. It employs more than 250 associates. Members have access to a statewide network of more than 9,000 physicians, medical technicians, treatment facilities and hospitals.

Along with its affiliates, LifeWise employ more than 3,000 associates and has a membership of nearly 1.6 million throughout Oregon, Alaska, and Washington.


A Sampling of LifeWise Health Plans

The Wise Essentials Plan combines great value with excellent coverage. This plan offers consumers essential, affordable coverage and low monthly premiums. Annual deductibles vary from $1,500 to $10,000 with 25% coinsurance for in-network services. Prescription drug coverage, office visits, preventive care and alternative care options are all available to members choosing this plan.

The Wise Choices Plan offers the broadest coverage to consumers and provides different copayment and annual deductible options. Deductibles range from $2,500 to $5,000 with 20% copayment. The Wise Choices Plan also includes coverage for preventive care, office visits, emergency visits, and other services. Customers can also add Vision care to this plan.

The Wise Savings Plan gives consumers the opportunity to establish a tax-advantaged Health Savings Account (HSA) combined with a high deductible health plan. Members choosing this plan contribute funds to the HSA and those funds are then used to pay for qualified medical services and procedures throughout the year. The annual deductible for an individual is $3,000 and for a family it is $6,000. Both of these carry a 20% coinsurance.
