Plan Offerings
Empire Health Insurance Child Health Plus – This plan offers extensive benefits to children under 19-years-old and access to over 65,000 in-network provider locations and 147 hospitals throughout New York State and bordering counties in New Jersey and Connecticut.
Empire Health Insurance Healthy New York – This plan extends benefits to individuals and sole proprietors. It includes in and out-patient benefits, prescription drugs, and emergency services.
Empire Health Insurance Direct Pay HMO/POS – This plan offers the same benefits as a traditional HMO for in-network services with the added option of obtaining out-of-network POS coverage.
